How Jane’s NDIS Journey Was Transformed Through THC's Support Coordination

It’s well understood that navigating the NDIS is difficult for many participants, especially those from culturally diverse backgrounds. There can be challenges around language, communication and cultural nuances in support needs. 

Our team at THC understand how difficult these barriers can be and has a mostly bi-lingual team of Support Coordinators.

Jane, for example, has been receiving Support Coordination services from THC for about 16 months and it's led to significant changes in her life.

Until recently, Jane had always lived with her family and received all forms of support from her parents. Although she had access to healthcare support, she didn't have access to any therapeutic support, in-home supports and other necessary supports. Julian Kim, one of our Korean speaking Support Coordinators was assigned to Jane to help her navigate the NDIS.

Working with Jane and her family, Julian sought an urgent review of Janes circumstances, requesting Specialist Disability Accommodation (SDA) and Supported Independent Living (SIL) funding, with the objective of transitioning to supported living.

Our Support Coordinator, Julian Kim, has been immensely caring and helpful in guiding Jane and our family through a successful transition to Supported Independent Living,said Moon, Janes father.

Jane was successfully approved for SIL and SDA funding. Six months ago Jane moved into her new home in Dee Why, which has been a significant achievement for her. She has formed new friendships with other NDIS participants and her dedicated support network. Moreover, Jane is now engaged with a local Community Hub which she attends during the week and has established connections with local health professionals.

We have noticed that Jane has been getting more independent and outgoing,Moon said.

At her new home in Dee Why, Jane enjoys watching movies, singing and dancing, playing with her fellow resident's cat, and going out to restaurants.In addition to Janes social network growing, she now has access to regular speech therapy, occupational therapy, physiotherapy, dietetics, and orthotic services.

Thanks to THCs complex and extensive Specialist Support Coordination, Jane has acquired a new wheelchair and other essential assistive technology (AT) equipment. Support Coordination is an essential service that can help participants and their nominees explore opportunities that help participants live independently and receive the supports that matches their needs.
